Required Identity and Authentication of a Petition

A petition must bear the petitioner's full name, Oracle Employee Number, signature and address. Where it is written by a person other than the petitioner, the writer's signature and address must also be included. Where a person submits a petition on behalf of an illiterate person, the petition should state that the writer has been asked to write on behalf of the illiterate person.
